Abstract
Purpose: To investigate feasibility and clinical utility of ultrasound (US)-guided diffused optical tomography (DOT) in differentiating malignant breast lesions from benign lesions.

Materials and Methods: 37 women with 40 lesions performed US on May 2008 were included in this study. Conventional US was used to locate the breast lesion, and DOT to measure the total hemoglobin concentration (THC) for each lesion.

Results: There were 28 benign lesions and 12 malignant lesions. The maximum THC of all 40 lesions was calculated from DOT. The average maximum THC in benign lesions was 180.9¡¾11.6 ¥ìmol/L, and in malignant lesions 289¡¾14.1 ¥ìmol/L. The maximum THC of malignant lesions was significantly higher than that of benign lesions (p = 0.015). When a maximum THC of a 180 ¥ìmol/L was used as the cutoff value for differentiating breast cancer from benign lesions, the sensitivity, specificity, positive predictive value, and negative predictive value were 91.7%, 53.6%, 45.8%, and 93.8%, respectively.

Conclusion: US-guided DOT, a noninvasive functional imaging, has potential utility for differentiating malignant breast lesions from benign lesions.
